Although conversations about men's health often reduce male insecurity to what hangs between the legs, I never really thought much about my penis growing up. I knew the jokes. In my Xavier School days back in high school, I heard the locker-room conversations. I scrolled past the memes equating masculinity with inches. But none of them stuck.

Not until I was nineteen.

My first boyfriend unknowingly introduced me to a different kind of self-consciousness. It wasn't because he mocked me or made me feel inadequate. In fact, he never did. But the difference between our sizes quietly settled inside my head. During intimate moments, I found myself wondering, Would he be more turned on if I were bigger? Was I enough?

The irony is that I have always believed that size does not matter.

Even with my own sexual partners, I've always stood by the idea that good sex has very little to do with measurements. It's about chemistry. Communication. Mutual pleasure. The best lovers I've had were never defined by what they carried inside their underwear but by how well they listened to another person's body.

Still, believing something intellectually doesn't always silence insecurity emotionally.

There is a difference between what you advocate and what you secretly question about yourself at three in the morning.

Fortunately, or perhaps coincidentally, I used to work as a Senior Receptionist at Hara Clinic, serving as the clinic's front-of-house. One unexpected privilege of working in sexual wellness is that curiosity becomes part of the job. Every day, I watch men quietly walk through our doors carrying questions they would never ask in public. Some worry about erectile dysfunction. Others want to address Peyronie's disease, improve sexual confidence, or simply understand what modern sexual wellness can offer.

One treatment that repeatedly came up in those conversations was the P-Shot, formally known as the Priapus Shot.

Despite the intimidating name, it isn't a filler or an implant. The procedure uses platelet-rich plasma (PRP), prepared from the patient's own blood. After drawing a small blood sample, it is spun inside a centrifuge to isolate the platelet-rich plasma, a concentrated portion believed to contain growth factors involved in tissue repair. Once prepared, the PRP is carefully injected into specific areas of the penis after local anesthesia is applied. The entire procedure is relatively quick, and because the material comes from the patient's own body, the risk of allergic reaction is generally low.

Listening to patients describe why they wanted the procedure made me realize something.

Very few of them were chasing pornography. Most simply wanted to feel like themselves again.

Some hoped to regain confidence after aging. Others wanted firmer erections or to improve their sexual performance. The motivations were surprisingly ordinary, even vulnerable. Men's health, I realized, is rarely about vanity alone. More often, it is about confidence, intimacy, and the quiet desire to feel comfortable inside one's own body.

Ironically, if there was one thing I genuinely wanted to improve about my own penis, it wasn't the size.

It was my premature ejaculation.

For years, that bothered me far more than any tape measure ever could. I wanted better control. Better endurance. Better confidence, as a former hardcore pamintang bakla, not because I wanted to impress anyone, but because I wanted to enjoy intimacy without constantly worrying about ending too soon.

Eventually, I decided to experience the P-Shot myself.

The first thing that surprised me was how uneventful everything felt.

Before the procedure even began, the consultation was surprisingly thorough. Expectations were discussed honestly, and Doctor Deano of Hara Clinic PH walked me through it very professionally. There were no exaggerated promises or miracle claims but only an explanation of what the procedure could potentially improve, what it could not, and why individual results vary.

As someone who worked at the clinic, I already knew how seriously patient confidentiality is treated. Still, experiencing it as the patient rather than the staff member was different. Every step felt private, respectful, and professional. There was no embarrassment. No unnecessary exposure. Just healthcare delivered with dignity.

Then came the procedure itself.

Contrary to what most people imagine, pain was not the defining experience. But I must say, for almost an hour, parang nalaglag ang tite ko (due to the local anesthesia), and it felt like I was involuntarily peeing myself.

After the numbing medication took effect, I mostly felt pressure rather than sharp pain. The injections were brief. There were moments of discomfort, certainly, but nowhere near the horror stories people often invent whenever needles are mentioned near male anatomy.

In hindsight, the anticipation was worse than the procedure itself.

Recovery was similarly uneventful. One day was enough. There was minor soreness, but I was able to return to my usual routine without much interruption. I followed the post-procedure instructions carefully, allowing my body time to heal before resuming sexual activity.

What changed afterward was subtle at first.

My erections felt noticeably firmer. Admittedly, there were moments when they arrived at the most inconvenient times. I found that even the slightest stimulation, whether from a change in temperature, an accidental brush against something, or even a fleeting sexy thought, could trigger an erection. It was an unexpected adjustment. While it was reassuring to know that my body responded so readily, it also meant relearning how to navigate those spontaneous moments with a little more composure. More importantly, I noticed better stamina during sex. For me, that translated into improved confidence because I no longer felt like I was racing against my own body. The anxiety surrounding premature ejaculation gradually became less consuming.

Then there was the unexpected part.

Over time, I personally noticed that my penis appeared slightly longer and fuller in girth when erect. It wasn't a dramatic transformation worthy of internet clickbait, nor did it suddenly redefine my masculinity. But it was enough for me to notice, and enough for me to feel more comfortable with my body.

More than any physical change, though, what surprised me most was the psychological shift.

I stopped thinking about my penis so much.

Ironically, the greatest benefit wasn't becoming obsessed with size. It was finally becoming less obsessed with it. Perhaps that's the strange paradox of men's health. Sometimes, the treatments we seek for our bodies end up healing our minds instead.

Would I say every man needs the P-Shot?

No.

But, hear me out! I am on to something here. I do think more men deserve honest conversations about sexual health without shame. Whether the concern is erectile dysfunction, premature ejaculation, declining confidence, or simply wanting reliable medical information, seeking help should never be mistaken for weakness.

For years, we have told men to "man up" while simultaneously expecting them to remain silent about the very parts of themselves that shape confidence, intimacy, and identity.

Maybe real masculinity isn't pretending everything works perfectly.

Maybe it is sitting in a consultation room, asking difficult questions, and giving yourself permission to be cared for.

Because sometimes, the most meaningful improvement isn't measured in centimeters.

It's measured by finally looking at yourself without insecurity speaking first.
